For those that would prefer to complete a hard copy application, this can be printed out from the product disclosure statement. Medical declaration forms for existing health condition assessments and travel insurance claim forms are also provided below. Travel insurance policies are set up on the same day that application forms are received, and documentation forwarded to the traveller. Medical assessments are generally provided by Toursafe within 24-48 hours of receiving medical declaration forms.
