Product disclosure statements containing the policy wording for the QBE Travel Insurance plans available through 2020 DIRECTINVEST can be downloaded below. For those that would prefer to complete a hard copy application, this can be printed out from the product disclosure statement. Medical declaration forms for existing health condition assessments and travel insurance claim forms are also provided below. All completed applications and forms should be sent by fax to (02) 8273 2010.  Alternatively, forms can be forwarded by mail to 2020 DIRECTINVEST for processing:

Travel insurance policies are set up on the same day that application forms are received, and documentation forwarded to the traveller.
